Incidence of muscle atrophy by ultrasound in the intensive care unit

Abstract

Introduction: Muscle atrophy in critical care may lead to increased length of stay, as well as intra- and out-of-hospital mortality. Muscle ultrasonography of the lower extremities has been validated as a reproducible method in its evaluation

Methodology: An observational, prospective, longitudinal, and descriptive study in the intensive care unit of the Hospital General León, Mexico. All patients admitted to the unit were included, follow-up was performed with two measurements of the thickness of the rectus femoris (admission and on fifth day), atrophy was defined as a decrease of 14% in thickness, and the incidence was identified as the percentages with 95% CI. 

Results: A total of forty-three patients; the incidence of muscle atrophy by ultrasound was 42% (95% CI; 27% to 57%), the decrease in the thickness of the rectus femoris in the group with atrophy was 28%, and in the group without atrophy was 5.3%. Albumin correlated with rectus femoris thickness from day one (r = 0.55, p = 0.03); Likewise, underweight patients developed muscle atrophy (p <0.0001). 

Conclusion: Incidence of muscular atrophy in intensive care was high in the studied population.
