Size of critically ill patients and its relationship to mechanical ventilation and vasoactive agent dosing

Abstract

Anthropometric measurements such as height are performed to calculate weight in critically ill patients. This is crucial for precise medication dosing and tidal volume adjustment in mechanical ventilation. There is no consensus regarding the appropriate measurement technique, the correlation among the different anthropometric methods and their level of consistency with the real height of the patient, weight determined by formula and doses of drugs such as vasoactive drugs, and tidal volume. 

Objective: To establish the relationship between the anthropometric techniques used to calculate weight and the dose of vasoactive agents and tidal volume used. 

Methods: 35 critically ill patients on mechanical ventilation were included and assessed using different anthropometric methods for height measurement, compared against the method used in the unit. The variables analyzed were demographic data, weight, height, main diagnosis, vasoactive dose, and tidal volume. 

Results: There was no agreement between knee height, arm span and Smartphone measurements for height and weight; for vasoactive volume 1 and 2, agreement was found between the method used and knee height, whereas this did not hold true for tidal volume.

Conclusions: There is a correlation between the measurement used in the ICU with the knee height technique, and the calculated dose of the vasoactive to be used, but there was no correlation with tidal volume calculation, indicating a greater risk of developing mechanical ventilation-associated lung injury.
