Via oral no paciente criticamente doente: a primeira coisa em que pensar

Resumo

Embora a maioria das directrizes clínicas sobre nutrição no doente crítico falem sobretudo de nutrição enteral e parenteral, pouca importância tem sido dada à principal via de alimentação nos seres humanos: a via oral. Esta via não deve ser tomada de ânimo leve, e esta revisão narrativa apresenta múltiplos cenários de importância para a avaliação do início seguro e da adequação da ingestão alimentar no adulto gravemente doente. 

A importância particular que cada indivíduo atribui à alimentação, que é um veículo de sensações, memórias e sentimentos, não deve ser esquecida, para além da importância da via oral na fase de reabilitação da condição crítica ou pós-crítica. Esta revisão explora diferentes condições associadas à iniciação da via oral na UCI, a fim de orientar os clínicos na tomada de decisões sobre a sua iniciação, num contexto de igual importância como o cumprimento dos objectivos nutricionais, a fim de atenuar a desnutrição.
