Home parenteral nutrition protocol for adults

Introduction: Home Parenteral Nutrition (HPN) is an alternative form of nutritional support for patients who require it, helping to improve their quality of life and autonomy. This nutritional intervention requires standardization of practice based on the best available evidence. Objective: to provide guidelines, adapted to the Colombian context, for the appropriate and safe provision of HPN in adults. Methods: a directed literature review was conducted by searching databases (Pubmed, Medline) as well as clinical practice guideline developers and compilers, using the term "Home parenteral nutrition" in English and Spanish. The search was limited to the last 5 years with a cutoff date of May 2024. The highest-quality guidelines were reviewed and selected, evaluated using the AGREE II (Appraisal of Guidelines for Research and Evaluation II). The recommendations extracted from the evidence were adapted to national practice by a multidisciplinary group of professionals with expertise in the subject. Aspects relevant to implementation were analyzed. Results:Three clinical guidelines were selected as the evidence base. After adaptation, the recommendations were grouped into key areas: basic principles, indications, safety criteria, use and care of devices, infusion lines and catheter sites, nutritional mixtures, monitoring, and patient and caregiver education. An algorithm was developed to assist with implementation. Conclusion: adherence to standards of care for the use of HPN by multidisciplinary teams, along with effective evidence-based interventions, facilitates the implementation of safe and effective HPN.
