Prevalence of patients at risk of developing refeeding syndrome: a cross-sectional descriptive study

Abstract

Introduction: Refeeding syndrome (RS) is an acute, complex, and lethal metabolic disorder that occurs as a consequence of the abrupt reintroduction of nutrition after a period of caloric restriction or prolonged fasting. It is generally underdiagnosed due to the lack of consensus on its definition. The alterations that RS presents are characterized by metabolic and electrolyte imbalance, which can manifest in a variety of degrees. Its prevention is carried out through early screening, evaluation and monitoring of the population at risk.

Objective: To describe the prevalence of RS risk at hospital admission in adults, according to sex and age, hospitalized in the services of the “Hospital Interzonal General de Agudos (HIGA) Professor Dr. Luis Güemes” during the months of January to April 2022.

Method: Observational, descriptive and cross-sectional study carried out on adult patients of both sexes who were hospitalized in general wards and intensive care unit (ICU). The variables to be studied were: Prevalence of RS risk according to the National Institute for Health and Care Excellence (NICE) criteria, age, sex, Nutritional Risk Screening (NRS-2002) and body mass index (BMI).

Results: The sample consisted of 333 patients, 7.2% of whom were at risk of RS. The service with the highest prevalence was surgery with 38%. The prevailing criteria for the identification of RS risk, according to NICE guidelines, were low or no intake for more than 10 days, followed by weight loss of more than 15%.

Conclusions: The protocolization of nutritional assessment and the detection of RS risk by NICE criteria tool is essential to avoid underdiagnosis of this entity. It will also allow the implementation of appropriate measures to prevent its development (water, electrolyte and vitamin replacement, and progressive initiation of nutrition).
