Changes in thigh muscle during hospital stay in critically ill patients: a systematic review of the predictive validity of ultrasound

Abstract

Ultrasound has been shown to be a useful tool for measuring and quantifying changes in muscle thickness during intensive care unit (ICU) stay. Loss of muscle mass in critically ill patients has a negative impact on clinical outcomes and is associated with complications that increase morbidity and mortality. Loss of muscle mass and function (sarcopenia) is prevalent in older adults and in the ICU. The objective of this systematic review (SR) is to analyze changes over time and to document clinical outcomes associated with such changes. The search was conducted in the PubMed, LILACS, Google Scholar and WorldCat databases, using MeSH, DeCS, and free terms. The selected studies were observational descriptive studies that included patients over 18 years of age in the ICU in whom ultrasound of the middle third of the thigh was performed. Data were collected by 3 independent reviewers using a standardized form and disagreements were resolved by consensus among all reviewers. Quality assessment was performed by two independent reviewers according to the Joanna Briggs Institute (JBI) checklist. Eleven studies were included, which reported the technique used to perform ultrasound as well as measurements of the muscle layer thickness (MLT) and cross-sectional area (CSA) of the quadriceps (Q), rectus femoris (RF), vastus intermedius (VI), vastus lateralis (VL), and vastus medialis (VM) muscles. Among the main findings in the studies, muscle loss was identified during the first 10 days of ICU admission, mainly on days 5 and 7, and this loss was associated with unfavorable clinical outcomes such as longer ICU stay, increased mechanical ventilation days and higher mortality.
